Hi @Riddhi Kshirsagar ,

Welcome to Atlassian community !!

To add custom field: https://support.atlassian.com/jira-service-management-cloud/docs/add-a-new-custom-field-to-a-project/

To add custom fields on customer portal :

Go to project settings-> Request types 

Here you'll see the agent view and request form, the request form is what customers see, you can add or remove fields as desired.

Hi @Riddhi Kshirsagar ,

In the project settings of your Jira Service Management Project you can configure the request types. The request types are the forms that are shown on the customer portal.
