You can use "ProForma: Forms and Custom Fields for Jira", this application is really getting traction on marketplace, and also staff picked solution.

@MG  I don't think so, as they are not creating custom fields.

But better you connect with ProForma team, I guess they are best to comment on this.

Briefly, it is now. ;)  Dynamic Forms for Jira. You will get features like:

• set value based on condition
• conditionally show/hide fields
• making them required or read-only
• set variables in text fields.
