How do I change the title of a transition screen?

I have a cloud instance with a few workflows. For one of these workflows, when I transition to "Resolve", the screen says "Resolve 2" at the top, and the submit button also says "Resolve 2". I can't find an easy way to edit this in the screens/workflows menus. How can I make this change?

It's displaying the name of the transition.  If you edit the workflow, find the transition and rename it, the screen's caption will change.

The above imagines are the workflow, transition, and the transition screen of an issue using the workflow. When I transition from a status to resolved, the screen says "Resolved 2" but the transition itself is simply called "Resolved"

There are two transitions there - check both the unnamed incoming transition and the "all"

The other transition is for when the approval is declined and it is called "Declined". The transition screen I posted was using the "all" transition as it was not from the approval step
