How do I upgrade Portfolio from 2.13.1 to 3.0?

Hi,

I am not familiar with the procedure to upgrade an app for JIRA. In this particular case, I would like to upgrade Portfolio from 2.13.1 to 3.0?

Is there any documentation online with the steps for upgrade an app?

Do I need to disable the app? Uninstall and then install the new version?

Needless to say, I don't want to loose any data ;)
