dynamic forms in JIRA (not in customer portal)

While creating a new issue, I want custom field "Field" to show up when option A is selected in a custom field called "Drop down". I cannot make it to work. I have tried dynamic forms plugin with no help. Anyone have step by step instructions to make this work? I have created two custom fields which I want to be dependent on each other but don't know how to use dynamic forms plugin. When I go to Add-ons , I only see option to convert CF to dynamic field under 'Dynamic Forms' Add-on. What is the next step after that?

I also tried plugin 'Extension for JIRA servicedesk" which works fine on customer portal but has no effect when agent creates ticket in JIRA agent portal.
