just to clarify: From what I understand, you want to identify issues where there's been no new internal comment in the past x days, is that correct?

as already suggested, this isn't currently possible in "plain" JQL. You may be able to somehow workaround this limitation via Jira Automation, but someone else would have to lean in with the details. Maybe this is a useful starting point.
Alternatively, if you are open to solutions from the Atlassian Marketplace, there may be more options. There's a number of apps that extend JQL by additional functions; not sure about this exact capability though. There are also apps that provide their own, "non-JQL" searching and filtering capabilities. E.g., my team and I work on an app in which this should be trivial to do, JXL for Jira:
(For context, JXL is a full-fledged spreadsheet/table view for your issues that allows viewing, inline-editing, sorting, and filtering by all your issue fields, much like you’d do in e.g. Excel or Google Sheets. It also comes with a wide range of so-called "smart columns" that aren’t natively available, including the time since last internal comment, along with many other comment-related columns. As you can see above, you can easily sort and filter by the time list last internal comment, and also use it across JXL's many advanced features, such as support for configurable issue hierarchies, to issue grouping by any issue field(s), sum-ups, or conditional formatting.)
