Show a screen to set 'On hold reason' when transitioning ticket to 'on hold' status

I want a new screen when someone click on 'on Hold' how to do that can anyone guide me step by step process

People can choose to ignore to fill this field unless you make it mandatory for that transition.

In addition to above, you can add the validator of field required condition for this field. JIRA doesn't provide this capability. You may need to use a third-party plug-in like JSU or JWME.
