Add custom UI on workflow post function transition

I want to add custom user interaction screen on workflow transition. 

Is there any way to achieve it ? please refer sample UI below :

Hi @navya k - you can attach a screen to a transition and define the fields you want on the screen. But you cannot control the layout of the screen like your example. It is just a single column of fields.

Not without rewriting a whole load of Jira's code and recompiling it.

On a transition, you can give the users no screen, or a standard transition screen with any fields you want on it.

Anything else needs code.
