Identifying level using the mutli-level cascading select plugin with the lookup manager plugin

To solve a... unique issue, I am using the lookup manager to select a cascaded value from a custom field. Per the lookup manager's documentation, the values in a cascading field are identified by level, where the root level is null, 1st child is 1, etc. This plugin allegedly works with multi-level cascading selects but every time I try the lookup fails.  If I make identical two-level cascading select fields with the built in field type and the multi level type, the lookup works for the built-in field type but not multi-level.

So I assume its a syntax issue, but I can't find any info to support this, and don't know how to work around it.  Any thoughts?
