How to create intake form on Confluence for Jira issues

  • Design Your Form:
    • Add form fields: Choose from text fields, dropdowns, checkboxes, date pickers, and more.
    • Label fields clearly: Make their purpose obvious to users.
    • Utilize "Help Text": Provide additional guidance or examples if needed.
    • Consider Form Logic: If certain fields should appear only under specific conditions, set up those rules.
  • Generate an External Link:
    • Within Smart Jira Forms, locate the form you want to embed.
    • Navigate to the "Setting" tab and select "Share Form".

  • Create an iframe in Confluence:
    • Edit the Confluence page where your form should live.

We could add a more detailed tip within the guide:

  • Strategic Pre-Population: Consider which fields should ALWAYS be pre-filled based on the form context. On a support form, perhaps the "priority" or "assignee" field in Jira.

We've seen potential in bringing our Cloud add-on to Data Center. We're still in the early stages of gathering interest internally, but your perspective would be extremely valuable as we move forward. Maybe you can share some of your thoughts regarding this, so we could come back to the idea of development 🙂

I know that you've already contacted our support team, but I can duplicate reply here as well. 

The apps are not avaliable in Jira Product Discovery, however you can set Create Issue functionality to your Jira Product Discovery project. There are some limitations because the add-ons is not avaliable for Jira Product Discovery, so you can't view full form in the issue, but you can populate all form fields to map inside issue, so the data will be avaliable to view. You can also view all resposes inside add-on. So the steps are
1. Go to Smart Jira Forms add-on

2. Create form

3. Set up Create issue for Jira Product Discovery project and set up mapping for all necessary form fields that you wanna see.

4. Place your form on Confluence page

5. When the issue will be created you will see all resposes that was mapped inside JPD issue

6. If you want more info you can visit responses tab inside add-on, download responses and analyse with charts.

Thanks for all the DC requests - Smart Forms is finally there! Form builder, conditional logic and most features are live now. External sharing isn't available yet (same as Confluence), but we're targeting a release this month. Worth testing the core functionality to see if it works for your setup.  Before sharing will be released the forms are avaliable in issue submissions they can be add automatically or manually and act like a task templates or checklists with the functionality to map form responses to issue fields.
