On your reply to the comment by @Jack Brickey , no this is not an option.

Option you do have os the raise a request based on these options and then by use of an automation rule attach the right form that needs to be filled.

Thank you for your reply, what I actually need is when creating a Service Requests and there will be options of mandatory information (radio buttons) that user will need to select and when that combination information is selected, then the specific form (for the specific support group) that is met the condition will be showing for gather additional information that will be useful for the request.
