Thanks for your update.  Out of the box it is not possible.  You have the following options to achieve your ask -

1) Use existing "FORMS" configuration to create a form and map your form's element to the description field.  User will use the FORM to create JSM issues via the portal.  Here is the reference link on FORMS component associated with JSM out ot the box - https://support.atlassian.com/jira-service-management-cloud/docs/what-are-forms/

2) Take a look at the paid add-on "Scriptrunner for Jira" and use their "Behaviors" component -

Can you clarify on what you meant by providing the FORM in advanced in the Description field?

Are you trying to create a template setup for users when they populate the Description field?

꼭 이렇게 해야만 하는 이유가 있습니다.
