I am looking for a free plugin to add a dynamic form to my jira service management project(data center). Any suggestions guys?

You will have to pay for the app if you want any of the ones with more robust features. They are on server and not cloud. Proforma is the same tool used in cloud but has to be installed on server and costs money for the full feature set.
