Attach completed form to an automated email

What is the best way to attach a completed form to an automated email to be sent to a group of executives for review. I want to send an email response through Jira and attach the completed form. Is there another way to do it in lieu of copying all the form fields and custom ids? 

Example: What is your name (requestor)? {{Issue.customfield_10264}}
