Is there any way that I can report on the following field below and is there any relavence in the 'Field Key' field other than in calling JSON in API to report.

Hi @Rajeev Desai - Not without linking the field to a backend custom field.
It's important to understand that forms (as presently designed) are intended for providing guardrails for customers to ensure all data is collected thoroughly and consistently so that agents spend less time on triage activity. So, their focus is more on individual issue interaction than reporting across multiple issues.

On the right side of the screenshot you shared, there's an option for "Linked Jira Field". This is where you link the form field to a Jira field. If the field doesn't exist in Jira, you'd need to create one.
