Currently, it's not possible to add placeholder or hover text for fields in the customer portal. Based on your question, I created a feature request suggesting the implementation of this ability:

Please let me know as soon as Jira has this capability. We have a large instance of Zendesk and would like for Jira SD to replace it but the hover over capability in the fields in the customer portal is a must.
