Hi, everyone. I would love to be able to create a form or integrate with something like Jotform to turn procurement requests into service desk tickets. I've been searching around but haven't found anything on how to do that. Am I just completely missing how to do that?

You can create Forms in JSM, and then use them in Requests.
If you go to Project Settings (if you have permissions), then under Request Management, you will see Forms underneath, Request Types.
Have a look at the templates to see what is possible, as there are a few limitations, but overall we use them a great deal.

Thanks, @Mark Higgins . Can the forms be used by folks who aren't Jira Users? Basically, we want our front line leaders to be able to make requests, but they aren't going to be users within the Jira system. We just need a consolidated way to track the requests that are coming in.

What you are suggesting sounds like a Widget, that is again under Project Settings, but further down, and under Channels and Self Service.
I've never created one, but it may give you enough functionailty for what you are looking for.

You’re on the right track—and you’re definitely not missing anything. Jira Service Management (JSM) does offer native forms, but there are limitations when it comes to sharing those forms with non-Jira users or customizing them beyond basic structure.
If you are okay with add-on, you can also take a look at Smart Forms for Jira:
Smart Forms is a great fit for this cases. You can share the form via link or QR code, embed it on your intranet, and ensure all procurement requests flow into one place—your Jira queue.
Plus, you can apply field validation, use conditional logic, and map responses directly to Jira fields (like Priority, Approver, or Cost Code).
