Customizing Customer Satisfaction

I have a team that wants to do more customizing of the Customer Satisfaction survey.  They'd like to adjust the number of options for the user to select from (default only allows 1-5 with no ability to change the labels), and maybe having 2 or 3 questions.  Has anyone made changes like this before, or has suggestions for add-ins that do this?

You will need a third party plugin unfortunately.
