JSM 4.13 is related to Jira Core 8.13. Please take a look here https://marketplace.atlassian.com/apps/1222010/jira-cloud-migration-assistant/version-history in order to download the related JIRA migration assistant app version

Yes it is, you will need to manually install the Jira Cloud Migration Assistant.

but Server used AD for authentication login, not sure it's supported on the cloud
