Error when creating ticket 'Request a new account' in Jira

Requirement:

I wish to create the ticket 'Request a new account' in Jira.

Issue:

I receive the following error message after filling in all necessary information correctly: "Your request could not be created. Please check the fields have been correctly filled in."

In the 'Description', I typed the name of the person that needs the account and the e-mail address. After clicking on 'Create', I receive the error message.

Could you please look into this? Thank you in advance!

i suspect that you have a required field that is not exposed to the portal. Try to create one using the application and see if there is a field that is required. If so then go to project > project settings > request types, edit the request type and add the field.
