×We would like to add some custom fields to the form templates that customers use to submit a request. How do I create a custom customer request form in Service Management?

There is two ways of doing this, the first thing you should look at is the request form for the request type. It will show you what the customers sees on the Portal, and from there you can also add more fields if needed. Your second option is to associate a Form with the request type. The difference between Form and request form is that Form can have conditional sections that are only shown based on what the customer select in a previous question.
