How forms work for customer

Hi i want to have only one forms for my customer in my jira portal service dans when they select a problem its send them to a link that is a form and i want them to fill the form so it create an service request. 

i have a question what permission the customer need to have for them to create the issue from a smart link in confluence

have you tried to do this all with Jira native forms instead of third-party forms? it'll be much easier, I think
