Question: for trigger "Work Item Assigned", when automation looks at the ticket, does it automatically know that there's a form attached to that ticket?
Right now, I have trigger as Work Item Assigned and then if there's a specific form attached, then look into that form and see if the Select_Facility matches to Canonsburg. If it does then check to see if it matches the listed departments. If that matches, then it sends an email to the supervisor in those department.
So basically I want to send an email update for when a ticket is assigned to an agent but only to respective supervisor in those specific department. All of which are filled out in the form.

Assuming that your fields are correctly structured on your form, it seems like you could use the recently released form smart values to grab the needed info.
