You can use the out-of-the box customer satisfaction service in Jira Service Management, this will send an email to the Reporter once the ticket has been closed asking them to score a star rating on the ticket.
There isn't much customisation available for this but there are some ideas listed in this post

If you really wanted to create a feedback form/questionnaire you could create another issuetype called Feedback and use the Forms functionality in JSM to create a questionnaire. Then set an automation rule to create and link a Feedback issuetype to the closed ticket, copying the Reporter and notifying them.
This is pretty convoluted and you would lose the out-of-the-box Satisfaction reporting, but you could make a feedback dashboard to capture this. Just quite a bit of work.
Is there something in particular that the Satisfaction feature isn't achieving?
