How to get rid of the welcome message at the top of jsm form

Is there a way to get rid of the 'Welcome to the Digitalization Request Platform' that's at the top of the form when a user goes to fill it out? also to get rid of the 'Raise this request on behalf of' section ?

That seems like something that the creator of the form added. Can you edit the form and verify/remove? You should find the form under project settings Forms if Advanced forms are used. If it is the standard form this would be under the specific Request types.
