I want to report a bug after an E-form is submitted by a tester

This is an E-form for testers, so If the tester reported that the case didn't pass, how can I automatically report that a bug happened from the form

You will need to code your form so that when they fill in stuff that should go into Jira, it calls Jira over its REST API, telling it to create the bug report.
