Hi Team,
I am creating a request form in a service project. I have a requirement to add a note/comment between 2 fields. This is more like an instruction for the users who would use this form, but I see no options to make it happen.
Any help would be much appreciated.

You can do it using Forms for your Jira Service Portal. With forms you can add notes/infos as you see in Confluence macros. You can use these notes to display the message that you want in your request form.

I agreed with using FORM suggestion mentioned by @Fernando Eugênio da Silva . Out of the box, it is not possible as you will have to use FORM to achieve what you wanted.
Essentially, the FORM element takes over the look of your request type screen.
