I am trying to build a multiple question survey to collect feedback for completed requests. I am being told Jira only provides a one question survey. I then received an email from noreply@am.atlassian,com to fill out a survey form on how they did on my ticket. That's what I am looking for. What do I need to have access to a survey like that, one which I can modify to suit my organization's needs?
