Hi @Kuntal Pathak and welcome to the Community,

there is an excellent documentation for forms: https://support.atlassian.com/jira-service-management-cloud/docs/use-forms-in-jira-service-management/ 

You could also setup a Request Type without the use of forms and use custom fields instead.

Thank you Rebekka, I am new to Jira, is there a way to find out if my account is an admin? I do not see these form creation options.
