What is the best way to add an image to a custom JSM form? I just want to add one image to a certain request type.

Hi @Darius Fields , Welcome to the Community! Glad you're here.

Right now, there doesn't appear to be a way to add an image. I looked for the ability to add HTML, I tried dragging and copy/pasting an image onto the form, nothing worked. There *may* be a third-party app that allows you to do that (if that's an option), you can search the Atlassian Marketplace. Sorry for the bad news, it would be a good feature to have!

Hi @Darius Fields

You’re right — native JSM request type forms don’t currently allow you to display a static image inline. The built-in fields cover attachments, but those are for upload, not display.

If you want to add an image to guide requesters (for example, a diagram, chart, or product options photo), one option is to use Smart Forms for Jira (developed by my team). With Smart Forms you can:

  • Embed static images or even GIFs using the Embedded Content field.

  • Connect the form to a specific JSM request type so it only appears where you need it.

  • Or, use it in place of the native JSM form — keeping all field mapping, attachments, and conditional logic in one place, but more cost-effective, because add-on dont require paying for agents, to create and manage forms.

  • Or, add it as an extra form to the request after it’s created — which is useful if you want a safer, more controlled way to collect additional details without modifying the initial request form.

Example: For an equipment request, you could show pictures of available devices in the form so users choose visually instead of reading a list.

That way, you avoid HTML workarounds, and you can maintain the same form across multiple request types while still tailoring the embedded image for each case.

Happy to share a quick setup example if you’d like to see how it works.

@Darius Fields

Welcome to the Community.  For clarification - Are you trying to enhance your JSM Portal interface or Atlassian Forms?

For JSM Portal interface, I know this vendor Deviniti from Atlassian Marketplace has some great products to support the customization needs.  Example -

Theme Extension for Jira Service Management https://marketplace.atlassian.com/apps/1220300/theme-extension-for-jira-service-management?hosting=cloud&tab=overview

For Atlassian Forms - (where you can incorporate the Form into your JSM Request type form).  you can only add image to the Atlassian Forms by reference an image that is accessible on the internet.

Please advise, so we can assist you further.

Best, Joseph Chung Yin
