When setting up JSM approval you can choose if:
If you only have one approver and not a group, then the above are kinda obsolete, since you have only one approval. Read more here https://support.atlassian.com/jira-service-management-cloud/docs/add-an-approval-to-a-workflow/
There are plenty approvals apps to choose from the marketplace https://marketplace.atlassian.com/search?hosting=cloud&product=jira-service-management&query=approvals
To answer you question:
Also is it possible to take a note first before the approver approves the request?
Not per se. The approver can write a comment first and then approve/decline the issue. WHat is your need?

I`m not so good at JSM Automation, so I suggest you try out Business Process Manager add-on.
This tool allows converting chaotic processes into an automated workflow

Hi @Raymart Cruz ,
you can't do that using builitin features. My suggestion is to try https://marketplace.atlassian.com/apps/1211763/herzum-approval?tab=overview&hosting=cloud that has these settings and many others.
Hope this helps,
Fabio
