Hi I have an automation that clone an incident from a specific SM to a different one,
I noticed that forms are not been copied to the cloned incident.
Is there any way to fix it?

As an alternative to Automation that can be really complex and time-consuming to set up, I can suggest trying our app Elements Copy & Sync that will allow you to clone an incident with forms and other issue fields.

If you are willing to try a third party app to clone your incidents, you can try our app Deep Clone for Jira.
With Deep Clone you can clone your incidents including comments, attachments, work logs, and issue status. Additionally, Deep Clone also supports cloning attached forms across projects. Read more on that here.

Are you cloning to another project?
If this answer is yes, then no a from can't be copied, a form can only be copied within a single project.
If this cloned issue is in the same project, then;
When an issue is created utilizing the FORM setup that Save the Form as a PDF and attached to the issue.
Afterward, you can use automation to copy the attached Form to another project's issue as needed.
