Is it possible and the most efficient way to build a customer portal for them exclusively in Spanish, other languages won't work for the on Jira Service Management (Cloud)
The rest of the customers portals on the same site are in English.

Expanding on Rebekka's on-point information:
Should your team connect Confluence as a database, you could then also translate all content/suggestions displayed for your users in Spanish via third-party apps. For example, we at bitvoodoo created a data-privacy first app called Translations for Confluence, which provides you with this possibility, but of course, there are more in the Atlassian Marketplace.

You can change the Project's (and therefore the portal's) default language under Project settings > Language support
You can add more languages there also and it will remind you to add translations to any Request type and other texts in the portal. I would advise to add Spanish as additional language rather than adding it as default, if everything else is in English as well.
JSM will always try to show the portal in the customer's profile language (if they are required to login and therefore have a profile) or the browser's language. Otherwise it will use the default language.
Remember, that the Help Desk (the "Homepage" of all your portals) would probably still be in English as I guess that's your System's default?
