Hi team,

I am building an Investor Intake Form in Jira Service Management and I’d like to organize it into clear sections (e.g Eligibility, Alignment, Risk Profile). At the moment, I only see options for input fields (short text, paragraph, dropdowns, etc.), but I don’t see a way to add static content or headers that would help separate and label these groups of questions.

Is there a way to add non-editable section headers (or dividers) in JSM forms?

• Do I need to enable ProForma Forms or a specific setting to make this available?

• If not, what is the recommended best practice to create clean section headers in request forms?

Currently, the only workaround I see is using a text field with preset/hide, but that makes the form look a bit messy since it’s still treated as an input.

Any guidance on the correct way to do this (or confirmation if it’s not possible in my version) would be really helpful.

Welcome to the community.

You can set normal text in a form and use divider lines or . Use the default formatting options.

You don't need to use a field or section for this.

Right now, the built-in form builder only provides input fields (short text, dropdowns, paragraph, etc.), so there isn’t a true “static content” or “header” element to visually separate sections. That’s why using a text field as a workaround feels messy — Jira still treats it like user input rather than display-only content.

A couple of options to consider:

1. JSM 

• You can use Section logic to group related fields together. While this helps organize fields, it doesn’t create styled headers or rich content.

• Help text can also be added beneath fields, but it’s tied to the input rather than working as a standalone header.

2. Marketplace Apps
If you want more control, apps like Smart Forms for Jira (built by my team) allow you to:

• Add static headers, dividers, embed images or files and rich text content elements directly into your forms. (plus you can adjust font and size of your header)

• Use conditional logic to display whole sections only when relevant (e.g. show “Risk Profile” questions if a user selects “High Investment Tier”).

• Pre-fill form elements with Jira values and map fields so responses update Jira issues automatically.

• Even share forms externally with people who don’t have Jira accounts — useful if investors or external partners need to submit information, and no need to pay for JSM agents, cause it can work across Jira, JSM, JPD withour requiring adding agents or contributors.

As @Sebastian Krzewiński mentioned, unfortunately, it is not possible to add headers to a request type.

In JSM Forms, you can add sections and name them, however, from what I see, these names are not visible later on the portal side: 

I suggest checking the Bundled Fields feature from the Deviniti Extension for JSM , which allows you to name individual Bundled Fields, configure their subfields, and display them on the request type as sections:

Unfortunately at this moment this is not possible to achieve with JSM forms. Your workaround sounds good. 

IMO - solution like you described should be available in JSM.
