Here is the Scenario:
I need a forms link (Similar to Microsoft Forms) which I can share across different people. They should be able to access the link and fill out the form. Once a form is filled and submitted an issue should be created on my board. (All the content provided in form can go in summary no need to associate it with JIRA fields)
Please let me know how to accomplish this scenario
It seems that you already using the out of the box FORMS application component (a part of Atlassian JSM) and created the form.
Here is how to setup it up to use the Form to create issues in your JSM project -
1) Go your specific project - Project Settings >> Forms section
2) Click on "Edit" of a specific form configuration
3) Click on the Settings tab and enable the "Create new issue" option and associate the form to a specific request type. Copy the link URL to share with others.
4) Save the changes for your form

If your Form is setup for Jira Software application, the concept will the same. Instead of associating a form to Request Type (for JSM project), you will associate the form with an issue type used in your Jira Software project.

Can you advise what type of project that you have created your form from? (Is the project a JSM project?)

Project settings > Details section and you will be able to find the project type.

Based on your screenshot, you have created a Jira Software Project (team managed format type), you should still be able just edit the FORM and enabled "Create new issue" option to obtain the URL for creating issue using the FORM.
Just move the x to the right, the icon should turned green.
Are you not able to do so?

One idea I have would be to link your form to a request type. Make sure the request type is available in your portal and then share the link to that exact URL for the request type where the form is.

Looking at your screenshot above looks like you are using Jira Software not Jira Service Management.

I think so. The screenshots we showed you were Jira Service Management and seems like all of the functionality is not available in Jira Software. For example, I don't think Jira Software has portal capability.
