JSM Chatops Slack Channel Creation

HI

Im trying to have new slack channel created automatically when an incident is created. All set up is complete with JSM Chatops (AFAIK) and it looks like the channel is creating (I can see the channel name in JIRA) but when I search slack for the channel I cant find it. 
It also looks like it is not adding team responders as expected. 

Any ideas?
