Can't create Jira ticket getting an error "Couldn't send request"

When trying to create a Jira ticket I am getting the below errror:

 

"Couldn't send request
Your request could not be created. Please check the fields have been correctly filled in. We can't create this issue for you right now, it could be due to unsupported content you've entered into one or more of the issue fields. If this situation persists, contact your administrator as they'll be able to access more specific information in the log file."
I am not using any special characters and I have data in the required fields.  I have also removed the workflow, tested, same error. Re-created the workflow, same error.  Workflow is not showing any errors.
Thoughts?

Is this on just this project or other projects?  

I'm leaning on a field that is marked required on the project's field configuration that is not on the request form.  I would check that out.

This is just for on project that was working just fine and then all of a sudden this error appeared.

 

Where do I check that out?

If you're on a company-managed project, you would look at the field configuration whose scheme is associated with your project (gear icon->issues->field configuration).  Find those fields that are required.  Make sure the required fields are visible in your Request Form.

If you've never seen this before, you may need to find the JIra admin in your organization.

By chance do you have a screen shot of where required fields would show so that I can ensure they are on the form?

All of our projects have the same fields on the form and other projects work fine, so this is where I am struggling.
