I have a request type called 'Feedback' and linked to it I have a form that has the standard questions such as, 'how was your experience', which has 5 radio boxes.
How can I create a report in a dashboard to list all of these Issues, but also create a graph based on the 'how was your experience' question within the linked form?
End goal, Id like a dashboard graph that shows how many feedback forms have been completed, and a count of the answers given to the question in the form?

You can do it in two ways:
If you are not familiar with smart values, JSON, etc I would recommend to go for route 1.
Im not a fan of option 1. I can picture our custom fields reaching the thousands over time, especially if it becomes normal practice for other teams too.
Ill probably end up using this method so thank you, but will look into option 2 for long term solution.
