Hello,
I have attached a Form under Request Types that is to be filled out by a user. I have left the Form fields unlinked to the Jira fields as I do not want any duplication of information. The issue I am facing is that the field headers still show in the issue, and I would rather have it not show up at all. For example, 'Description' still shows up as a label/header in the issue without any user-filled information since I have left the fields unlinked despite removing it from Jira fields. Since all the information is already contained in the submitted form, I do not want any additional information in the issue.
Is there anyway to remove these unlinked field headers?

Welcome to the community. In addition to @Mikael Sandberg stated, if you don't have the FORM elements created in your FORM configuration design, then you will not see the elements to be displayed in your Request Type setup display in the portal.

I do have the FORM elements created in my configuration design, and I am successfully retrieving the information via the submitted attached form. I would like to keep the elements in my FORM but would not like to see the elements show up in the issue as headers.
I've updated my main post with screenshots to confirm that the fields are not currently part of the Request form, please let me know what you think.

Thanks for your images updates. It seems those fields elements are defined in your FORM configuration. If you remove them, then when one creates the issue, then the FORM section will not display those elements at all.

My conundrum is that I would like to keep all the FORM configurations as they are eliciting information as expected, however would just like to remove it from showing up again in the issue; I would only like it to appear in the attached form and I would like 'Steps to reproduce.. Business Impact..' to not be visible under the reporter who raised the issue.

Thanks for your further clarification, based on my experience that unfortunately you cannot hide those FORM fields from the reporter since he/she uses the FORM to submit their asks from the Portal UI.

If you go into the request type and the Issue view tab you can remove the fields you do not want to see by dragging them from the middle to the right hand side. Once you are done doing this, just save your changes.

I have already done this - I've removed all the fields possible; the Summary field is the only one remaining as it is mandatory.
Despite this, when a user submits from their portal, the issue still displays these hidden fields.

If they are only being displayed on the portal then go to the Request form tab and remove them from there. Any field that is listed on the request form will automatically be listed on the issue view.
