We want the assignee for a change to be added in CC when an email goes out for approval on that change TO an approver.
Currently when i go to edit the approval email template, i see only option TO, there is no CC on this template that i can assign to assignee.
While it's not possible to do this with the native template, you can certainly achieve it using Automation by creating a custom notification and including the Assignee in the CC.

I dont want Option 2, because the main reason for this requirement is for Assignee to be on same email that the approve gets, for follow ups etc.
I can try option 1, but if i disable this, can i put the approve and declines button within the email body from automation? Do you have an example

There is an open suggestion to add this as a smart value Add {{approval.buttons}} smart value to add service desk approval buttons to emails sent via automation rule
