I just wanted to know whether how to Auto fill of email id, names phone number and etc. In Jira form when external customer access the form to submit any change request, is there any inbuilt feature we can use in Jira service management or need to use third party APPS or extension to auto fill the required things.

Can you be more specific? What is it that you are wanting filled in? If these are ongoing clients you could enter them into the Assets module (would require a Premium subscription) and retrieve data about them from there. Other than that, this information is not stored in Jira.

I will give an example for this scenario,
Company have clients and they will fill the change request form whenever changes needs to be done which are external customer or clients not internal, so while they access the portal they need to login to portal link which is provided by us, so once they login field like Email ID, phone no and name should be auto filled so that customer need not to enter these fields.
so for above scenario is there any possibility in Jira service management where we can configure or need to use any third party app which are available in Jira?

You would need to implement something like the Assets module to retrieve stored information like. It is not included with regular user IDs.
