I would like to be able to invoke another form depending on selection in parent form.
The idea is to create an incident entry form for all problems users have, when they i.e. select notebook a form for notebook issues is invoked and the get specific guidance there.
The idea is to have each service owner, like Notebook support service to own and maintain their own forms.
Is that possible withe the current from builder of JSM premium or do we need an add-on??

You could do this after submission of the parent form, e.g. create an automation rule triggered on form submission, with conditional logic based on the values selected in the original form which would attach a new form.
Perhaps elaborate a bit further on your requirements, but it sounds like something along those lines should work.
Question would be if you want every service owner in a single work item, or if it might be better to create seperate work items and link them together.

Right now, Jira Service Management’s native form builder doesn’t support invoking one form from another based on a field selection. You’d typically need to pack everything into a single form or redirect users manually — which isn’t ideal for a modular setup like yours.
That said, you might want to check out Smart Forms for Jira. With it, you can:
✅ Embed one form inside another using logic
✅ Use conditional sections that display different form content based on user selection
✅ Allow each team to build and maintain their forms independently
✅ Attach multiple forms to a single issue — perfect for incident + follow-up flows
It works across JSM, Jira Software, Jira Product Discovery, and even in Confluence if needed.

As far as I know, no, you're not able to 'nest forms'. This means you cannot call different forms in a specific section of this so-called parent form.
Basically, you'd need to create a conditional section in the 'parent form' and recreate all those fields that you have in a different form.
You could try checking if there's an open feature suggestion related to your requirement here.
As for addons, I haven't been constructing anything like that, so I don't know for sure if any app supports this use case.

Seems I am not able to open a open feature suggestion on that page, I am only able to select JSD for Datacenter in the projects list.

You'll need to reach out to Atlassian Support for creating new bugs/feature suggestions. You can use "Technical issues and bugs" category. Also, feel free to point them to this discussion.
They usually make one additional search for open items, and if there aren't any, they quickly create one.
Btw, what Bob said might be some kind of workaround.
