I would like to know if is possible to add more columns to this request preview screen in the portal, such as SLA and others fields.

By default, Jira Service Management offers only fields which you see on the screen. If you need to display more fields, you have to use an app: My Requests Extension for Jira Service Management. Full list of supported fields you can find in our documentation.
