 We would like to use jira service desk and embed request forms into our website .Would like to know the options available.

if you use Jira Service Desk cloud, this is quite easy to achieve without any addon as shown here.

Thanks @Alexis Robert for your response .We would like to use the portal URL from our website. Is that possible for the Jira Service Desk via cloud.
