Reporter field showing 'Anonymous'

Hello,

Within one of our project desks in Jira, occasionally, when an issue is created, it will show the reporter as 'Anonymous'.  Not sure why this is and it appears to be stemming from one specific department upon issue creation.  Any thoughts on why this is occurring?

Hi Tori:

Can you confirm that all issues created in your JSM projects (especially for the one in question), that they are all created via the Portal UI?  Or, there are other setup where intake of issue creation is taken place (i.e. via Confluence, Custom REST API etc..)

I figured out the issue finally.  It was related to a field on the form I used.  I removed the field which was a requestor field as it was competing with the 'raised on behalf of' field on the form.
