Any change request that gets declined remains on the open list and isn’t closed off.
Looking at the workflow there is only Approve and Decline and both should result in closure. But only approval does and gets an automated Done resolution. Am I missing something here?

What is your transition set to for when the approval is declined? You may need to change the transition for when its declined. Let me know if this helps.

There are no outgoing transitions from this status. and the same for Request Approved but however Request Approved changes automatically sets the resolution to Done when approver Approves the request.
I need exactly the same doing for Request Declined and set the resolution to Done.

I'd recommend adding this post function to the transition.

Check the properties on the transitions, it sounds like there is no post function setting the resolution on the Decline path.
