When creating a form in the "New forms", how do you get it to the customer portal ?

I've created a Form using the new option and cannot get my jira instance to see the form. I am not quite sure where/what I am missing.

In the form, go to settings. From here you can enable the form to be visible on the portal and for which request types.

I've done that and I keep getting this error.....

‘Required’ field is not linked

Link a field in this form with the Employee Name Jira field in the Security Change with Approval portal form.
