Welcome to the community. You will just need to add the field to the Request Form tab screen. To access it, here are the steps -
1) Project settings >> Request Types
2) From the Request Types screen, click on the specific request type name to access the request type screen
3) Click on the Request Form tab and add the field by dragging it from the right pane to your request screen
