1. how to add new columns in Request Form, say Assignee, Created Date and Due Date?
2. when user sent an email to Jira Service Desk project, new incident is created based on this email but body of the email is not aligned properly in the Incident. How do we solve this problem.
In request form, you cannot add Assignee field. It comes under hidden fields for request forms.
Due date can be added. Just add due date field in Create ticket screen in jira, then select from request form.
Created date actually is displayed after the ticket has been created. You can view in Details.
