how to create specific forms like "i want to add a new functionality in my website"

hello

I'm searching details on this functionality in Jira : I need to make specific forms that my clients could use to ask me software evolution/new functionality.

i need to make a form with my specific questions, and i need to have the liberty to manage the text blocs, the radio list, the subjects, etc...

could you help me in making this?

You can definetely make the forms in JSM. You can find the full guide here: https://support.atlassian.com/jira-service-management-cloud/docs/use-forms-in-jira-service-management/

Also, are your clients outside of your Atlassian organisation? If yes, you would need to enable your JSM project to enable the users who don't have the Atlassian license to access your forms.

What you can also do is also to specify the questions and make your forms more dynamic by defining the sections in your forms (section start/section end). Here you can specify the condition on which part of your form will show to the client based on the option they have selected. Yes, you can add radio buttons, checkboxes and more.
